Salary:  £24,000

Location: Gloucestershire/Hertfordshire/ Oxfordshire

Hours: 37.5 hours per week

The Here For You is a pilot project part funded by the National Lottery Community Fund and provides specialist advice and support. This role will also work closely with the local Guideposts services in each county. 

As the Support Broker you will enable people with long-term health conditions to live their best life by helping them plan their best day, tailoring support and services to achieve it and getting the right people involved. 

Through our service we aim to ensure:

  • More people with long -term conditions access the information, local support and networks they need to live well in their communities.
  • Greater support for families and /or carers, resulting in reduced impact on their own health and wellbeing and ability to work or undertake meaningful activities.
  • More local support providers/volunteers and employers are enabled in effectively and successfully supporting people affected by long-term conditions.

As a Support Broker you will assist individuals in building and maintaining strong teams that help them to live the life they desire. The team around the person typically includes a combination of family, friends and paid supports.

We are looking for someone who:

  • Has knowledge and experience of the issues facing people living with long term conditions
  • Uses a strengths-based approach to increase people’s confidence to take control of their health wellbeing
  • Builds trust and relationships with people
  • Gives people time to talk about what matters to them
  • Actively listens to understand what matters to people from a holistic perspective
  • Co-creates solutions with people
  • Enables and supports access to solutions
  • Facilitates joined up care and social inclusiveness
  • Is knowledgeable about the range of local support available to help with people’s wellbeing issues
  • Has a social or health care background
  • Has proficient IT skills, can engage in web based research and is able to interpret and present information

A DBS check is required for this post. A full driving licence and use of a vehicle is essential as travel across the county will be required
